Job Overview
We’re hiring Care Navigators to join our Guiding Cares support team in South Miami. In this role, you’ll guide patients and families through their care journey—offering support, sharing helpful resources, and keeping everyone connected. Ideal candidates enjoy meaningful conversations, are highly organized, and have experience in healthcare or working directly with patients or families.
Duties
- Serve as the primary point of contact for patients and caregivers
- Provide regular check-ins and help coordinate care and services
- Share program resources and help patients stay engaged
- Maintain accurate records of patient interactions
- Work collaboratively with the broader care team to ensure consistent support
Skills
- Experience in a healthcare, caregiving, or customer service setting
- Strong communication and follow-up skills
- Empathetic, friendly, and reliable
- Bilingual (English/Spanish or English/Creole) is preferred for some roles
- Comfortable using digital tools to track outreach and document care
Job Types: Full-time, Part-time
Pay: $18.00 - $21.00 per hour
Expected hours: 25 – 35 per week
