Hybrid: Travel throughout The Hudson Valley area is required*

Hourly Pay: $25.00 - $26.00

We specialize in helping seniors thrive, especially those living with dementia and complex medical needs, by combining expert pharmaceutical care with a patient-first approach.

We’re growing fast in Hudson Valley, NY and looking for passionate people to join us as a Care Navigator. If you love connecting with patients, solving problems, and being part of a mission-driven team, we want to hear from you!

What You’ll Be Doing:

• Be the go-to person for patients and families throughout their care journey.

• Create personalized care plans alongside our awesome team of providers.

• Help coordinate services like medication management, lifestyle support, and follow-up care.

• Keep a close eye on patient progress and make sure care plans stay on track.

• Advocate for patients and make sure their voices are always heard.

• Provide education and support to help patients and families feel empowered.

• Collaborate with providers, social workers, case managers, and more.

• Schedule appointments, referrals, and testing so nothing slips through the cracks.

• Keep accurate records of care plans and patient progress.

• Stay up to date on best practices and continuously look for ways to improve care.

• Help train and mentor new team members as we continue to grow.

What We’re Looking For:

• You’re at least 18 years old.

• High school diploma or GED required.

• Valid driver’s license.

• Medical Assistant diploma a big plus.

• Experience in patient care or case management is highly preferred.

• You’re organized, compassionate, and have great communication skills.

• You think on your feet and know how to problem-solve.

• You genuinely care about helping seniors live their best lives.
