Please read description thoroughly:

-Currently looking to hire for Columbia and Spartanburg area’s.

-This position is primarily remote but will require case manager to complete home visitations a few times out the year.
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop appropriate service plans
- Collaborate with other professionals and agencies to coordinate services and resources
- Advocate for clients' rights and access to necessary support systems
- Monitor client progress and make necessary adjustments to service plans
- Provide crisis intervention and support during emergencies or difficult situations

-Complete required state mandated training

-Required to handle minimum caseload of 30

Experience:
- Bachelor's degree in social work or a related field preferred
- Minimum 2 years of Case management experience
- Case Management certification with minimum 2 years of assessment care planning experience.

Skills:
- Excellent communication skills, both verbal and written
- Strong organizational skills with the ability to manage multiple cases simultaneously
- Compassionate and empathetic approach when working with clients in challenging situations
- Ability to build rapport and establish trust with diverse populations
- Proficient in using computer software for documentation and case management purposes
