  • About the role

    At Western Health, we are committed to delivering the healthcare of the future while providing the best care for the people of the West. We are seeking an experienced and motivated Nurse or Allied Health Professional to join our Bed Based Transition Care Program (BB-TCP) at Williamstown Hospital.

    The Case Manager role plays a pivotal role in supporting older people transitioning from hospital to the next stage of their care journey. Working collaboratively within a multidisciplinary team, you will coordinate complex care needs, facilitate discharge planning, and support consumers and their families to navigate community, aged care and residential care services.

    This is an exciting opportunity for a senior clinician who is passionate about aged care, care coordination, patient advocacy and improving health outcomes for older people.

    Key

    responsibilities

    • Undertake comprehensive holistic assessments of patients and their care needs.
    • Coordinate complex discharge planning and transitions of care.
    • Develop and monitor person-centred care plans and client goals.
    • Facilitate access to community supports, aged care services and residential care pathways.
    • Collaborate with multidisciplinary and specialist teams to achieve optimal patient outcomes.
    • Provide expert clinical leadership, advice and support.
    • Contribute to quality improvement initiatives, evidence-based practice and service development.
    • Advocate for patients, families and carers throughout their healthcare journey.

    About you:

    • Current registration as an Allied Health Professional or as a Registered Nurse with AHPRA.
    • A completed postgraduate qualification (certificate/diploma).
    • A commitment to Western Health Care’s Values, Compassion, Accountability, Respect, Excellence, Safety.
    • Experience working in a senior clinical role and a strong capability for providing healthcare education and mentoring.
    • Ability to work autonomously with minimal supervision, while actively collaborating across multidisciplinary teams to deliver best patient care.
    • Equipped with high-level verbal and written communication skills, alongside strong negotiation and analytical problem-solving abilities.
