At Bridge Prenatal, we are a dynamic, fast-growing provider of in-person and virtual Enhanced Care Management services for pregnant and postpartum individuals. Bridge Prenatal offers a comprehensive approach to care during pregnancy and up to one year after delivery, addressing both medical and psychosocial needs of eligible Medi-Cal members.
Bridge Prenatal’s free services in southern California include: prenatal and postpartum education, mental health assessments, blood pressure checks, lactation and doula support, registered nurse evaluations, and referrals to community-based resources and social support services.
Bridge Prenatal is looking for a Case Manager to coordinate and manage care for expectant and postpartum mothers to promote maternal and infant health and well-being. Perinatal CMs serve as “resource companions” for engaged ECM patients. This role works alongside Social Work (SW) team to assist with care coordination, advocating for services, facilitating connections to community resources, and providing education and support.
Key Responsibilities:
- Reviews patient’s case by reviewing chart, MCP portal, and background demographic information including SW documentation and relevant validated screening assessments (SDOH, IRA, CHA, BAM, EPDS assessments, etc. per MCP guidelines)
- Develops individualized care plans based on assessment findings and patient input
- Connects patients with appropriate resources, including mental health services and substance abuse treatment if needed. Utilizes available technologies to efficiently coordinate services
- Coordinates access to social services such as housing, food assistance, childcare, and transportation
- Provides regular follow-up and support to patients, adjusting care plans as necessary.
- Monitors patients’ progress and addresses any barriers to achieving their goals
- Maintains accurate and confidential records of patient progress in various electronic health records
- Required to do in-home and virtual visits at the preference of the patient. Serving patients in either San Diego County (willingness to drive to South Bay), San Bernardino County, or Riverside County.
Qualifications & Skills:
- Associate or bachelor’s degree required
- Prior experience in perinatal health, social work, case management or employment at community resource centers or other related fields is preferred
- Strong clinical skills in assessment, intervention, and treatment planning.8.4 Ability to prioritize and manage multiple tasks effectively.
- Excellent communication and interpersonal skills, with the ability to work empathetically with diverse populations (bilingual preferred).
- Knowledge of community resources and systems relevant to perinatal care.
- Ability to work independently and part of a multi-discipline team.
Job Types: Full-time, Part-time, Contract
Pay: $24.00 - $30.00 per hour
