JOB SUMMARY: Responsible to assist in the coordination, planning and provision of spiritual support and counseling services to patients, families, and caregivers in keeping with their belief systems. Acts as a liaison between local clergy and the interdisciplinary team.
REPORTS TO: Executive Director
QUALIFICATIONS:
- Ordination or commission by a recognized faith group and experience in pastoral counseling. Experience with death and dying with individuals and families. Hospice experience preferred
- A minimum of one unit of Association of Clinical Pastoral Education, Inc. (ACPE)-accredited CPE Georgia
- Ability to solve practical problems; variety of variables with limited standardization; interprets instructions.
- Ability to communicate well, both verbally and in writing
- Ability to perform activities such as viewing a computer terminal, reading data and figures, visual inspection of patients.
- Valid driver’s license, personal vehicle for on-the-job transportation, and evidence of insurance coverage as required by company policy.
- Required to have visual acuity to operate motor vehicles.
JOB CONDITIONS AND WORK DEMANDS:
- Requires minimal lifting of 10 lbs occasionally and up to 10 pounds of force constantly to move objects.
- Prolonged periods of sitting with periodic kneeling, standing, stooping and walking, as well as communication by phone and in person.
- Travel will be required, by car.
- Both inside and outside environmental conditions.
- Work Monday through Friday may include on-call work weekends, evenings, and nights.

Occasionally – Exposure to microbial bacteria and other infectious agents inherent to care of ill
patient
Frequently – Exposure to fumes, odors, dust, poor ventilation, or smoking
RESPONSIBILITIES: (Included but not limited to):
- Assesses and identifies the patient/family/caregiver spiritual status and needs.
- Develops plan of care in coordination with Interdisciplinary Team
- Provide spiritual counseling services, without imposing any value or belief system on the patient or the patent ' s family unit.
- Completes initial comprehensive assessment visit within 5 calendar days of admission unless visit declined by patient.
- Document visit record electronically within 24 hours of the visit.
- Provides pastoral and spiritual care services to patients/families/caregivers.
- Collaborates with spiritual colleagues as appropriate.
- Participates in interdisciplinary team meetings.
- Consults and collaborates with other members of the interdisciplinary team in the development and
- updating of the patient care plan.
- Demonstrates continued professional growth and development through participation in education
- programs and review of current literature.
- Assists with finding clergy to perform funeral, memorial or graveside committal services as requested by families and assists with planning for services.
- Assures that all documentation meets requirements for accuracy and timeliness, and reflects services provided, billing requirements, patient/caregiver needs, and confidentiality.
- Facilitating or co-facilitating family meetings
- Facilitating effective communication between patient/family and the IDT
- Advocating for effective team dynamics, including conflict management, wellness and self-care
- Assist with debriefing difficult cases or deaths within the IDT
- Adheres to all Agency, state and federal policies and procedures, laws, and regulations which are relevant to job responsibilities.
- Perform other duties as assigned.
