Qualifications and Education Requirements
- Staff must have a minimum of a bachelor’s degree, or master’s degree in a field of human services, or a registered nurse.
- Supervision and oversight by an individual with a minimum of a master’s degree is required.
- Staff must be properly credentialed by the agency’s internal credentialing committee.
- Knowledge of community SUD programs, referral agencies, and client resources.
- Ability to relate to people from diverse backgrounds with multiple individual and systemic barriers to success

Role and Responsibilities
As part of the multidisciplinary team, the assessor will:
- Conduct full comprehensive psycho-social assessment and collect demographic information
- Administer and score functional assessment instruments necessary to determine medical necessity and level of care is included in services.
- Perform evaluations to determine the needs, strengths, functioning level(s), mental status, and/or social history of clients
- Evaluate and identify situations that may be potentially harmful to the client
- Collect information for treatment and crisis plans.
- Document within the EMR in a timely manner
- Facilitate or lead training and meetings, regarding assessments, as needed
- Work as a team with other departments to ensure the quality of care
- Participate in all clinical supervision, training, and staff meetings
- Always follow organization’s policies and procedures
- Perform any other duties assigned
