Community Health Program Specialist

As part of our commitment to whole-person care, we collaborate with Community-Based Organizations (CBOs) across New York to deliver Health-Related Social Needs (HRSN) services directly to individuals.
Position Overview
The Community Health Program Specialist plays a crucial role in ensuring the seamless delivery of HRSN services to Medicaid members in New York. This full-time position requires experience in public speaking, creating and managing program work plans, and executing project workflowsf. Proficiency in technology, including project management software, Adobe, Excel, and related software is essential. Experience in CBO and/or Social Care Networks (SCN) is a plus.
The ideal candidate is adaptable, thinks critically, and works well with others across teams. Strong communication, cultural awareness, and a willingness to learn are key for connecting with diverse communities. Attention to detail, motivation, and flexibility to handle multiple projects in a fast-paced environment are essential.
Responsibilities
- Program Coordination:
- Provide project support to ensure timely completion of initiatives.
- Manage accurate and compliant enrollment processing for participants including working with dietitians that provide nutrition counseling sessions.
- Stakeholder Engagement:
- Collaborate with external stakeholders to address challenges and coordinate efforts professionally.
- Act as a main contact for individuals and partners, addressing questions professionally.
- Data Management & Reporting:
- Track key program indicators and update performance dashboards.
- Manage databases and analyze data to spot trends and improve processes.
- Create program reports that are clear and accessible to a broad audience.
- Quality & Process Improvement:
- Recognize and communicate best practices, emerging trends, and program issues.
- Define and implement new workflows to improve operational efficiency and service delivery.
- Maintain compliance with program guidelines, regulations, and deadlines.
- Customer-Centered Operations:
- Assist staff in refining and developing training materials.
- Work with leadership to maintain clear and accurate written materials.
- Develop detailed summaries and plans to keep projects aligned with workflows.
Required Skills & Qualifications
- Technical Skills:
- Proficiency in project management tools and CRM platforms.
- Expertise in Microsoft Office Suite (Excel, Word, PowerPoint, Outlook).
- Experience managing and analyzing data systems; ability to collect, organize, and analyze large datasets.
- Critical Thinking & Agility:
- Ability to think quickly, adapt to changing priorities, and implement solutions effectively.
- Strong problem-solving and decision-making skills under tight timelines.
- Organizational & Project Management Skills:
- Detail-oriented with the ability to manage multiple projects, track deliverables, and meet deadlines.
- Experienced in prioritizing work in a fast-paced setting.
- Ensures deliverables and program activities are on track using project management workflows.
- Communication & Interpersonal Skills:
- Excellent communication skills, with the ability to speak and write clearly to internal and external stakeholders.
- Proven ability to communicate across diverse backgrounds with cultural, linguistic, and educational awareness.
- Analytical Aptitude:
- Able to interpret data to identify trends and suggest improvements.
- Fluency in English and Spanish (written and spoken) preferred; other languages considered as needed.
Work Setting & Schedule
- Full-time; primarily daytime hours with some evenings and weekends required.
- Hybrid work setting; must reside and operate in the state of New York.
- Occasional travel may be necessary, up to 25%, typically within New York
