SUMMARY OF POSITION

The Day Habilitation Floor Lead reports to the Day Habilitation Supervisor. The floor lead directly supports individuals with developmental and other disabilities served through the Habilitation Services and is responsible for implementing individuals’ Day Habilitation Service Plan objectives and documenting their progress. The Floor lead will assume the responsibility of the Day Habilitation Supervisor in their absence. This position will be compensated on an hourly basis.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Ensure appropriate group ratios (max 1:7 within a group area)
  • Ensure staff is actively engaging participants by continuous supervision of multiple group areas.
  • Ensure attendance collection upon arrival to the program and all transitions between sessions.
  • Ensure incident reports are completed and submitted to Program Director.
  • Ensure all clients are being appropriately assisted in personal hygiene tasks according to level of need
  • Maintain the day program in a clean and orderly fashion. Assign cleaning tasks. Ensure completion by staff.
  • Ensure DHSP objectives are being worked towards daily and documented.
  • Train staff on specific DHSP/ISP goals and objectives.
  • Ensure individuals are spoken to in a respectful manner at all times.
  • Ensure staff and individuals are trained in Human Rights.
  • Ensure ongoing communication with Day Hab Supervisor, Service Manager and Program Director.
  • Maintain program within fixed monthly budget.
  • Implement agency policies and procedures as stated in the agency policy manual.
  • Provide programmatic coverage as needed through schedules prepared in advance using allotted FTE's with

oversight of Supervisor and Service Manager.

  • Educate and assist individuals with skill development.
  • Ensure bathroom and lunch lists are completed daily.
  • Ensure individuals have choice in their activities, goals, objectives.
  • Assist individuals to be the primary decision makers in their lives.
  • Oversee client accounts. Distribute emergency fact sheets and funds for community outings. Ensure

deposits/withdrawals are documented.

  • Ensure program maintains health and safety standards. Inform Program Director, Service Manager
  • and Day Habilitation Supervisor of any daily concerns.
  • Implement staff recognition and promote positive working relationships.
  • Direct staff to complete tasks to include: loading/unloading vans, transitioning between groups,

providing lunch supervision, and empowering them to be self-sufficient with down time.

  • Ensure staff are doing as assigned: Running groups effectively, no personal conversations, no cell phone use, earbuds etc.
  • Serve as programmatic supervisor to Developmental Specialists.
  • In the absence of the Day Habilitation Supervisor perform all functions of that position.
  • Implement Agency policies and procedures as defined in the Agency Policy and Procedure Manual.
  • Develop and implement appropriate and functional activities/curriculum.
  • Maintain individual data books to ensure daily data collection, current data sheets, and submission

to the Program Director for DHSP documentation.

  • Will serve as a direct support staff when needed to ensure appropriate ratios.

MARGINAL DUTIES AND RESPONSIBILITIES

  • Interact professionally and maintain effective relationships with all other departments within the agency.
  • Maintain confidentiality and positive demeanor while successfully juggling multiple priorities.
Requirements:

PHYSICAL AND MENTAL QUALIFICATIONS

  • Maintain First Aid, CPR, AED & Safety Care Certifications.
  • Ability to work a flexible schedule based on programmatic coverage, weekends, holidays, and program emergencies.
  • Good written and verbal communication skills.
  • Maintain trainings specific to individual programmatic issues.
  • Ability to plan and organize work and the activities of others.
  • Actions should reflect judgment that would not jeopardize the health and safety of the individuals.

MINIMUM QUALIFICATIONS

  • B.A. or B.S. preferred; Minimum High School Diploma or GED
  • Minimum one-year supervisory experience in Human Service or Education.
  • Must be 21+ years old, have a High School Diploma or GED, a valid driver’s license, daily use of a car on site (preferred but not required) and be available by telephone for daily contact.
