As a Designated Coordinator at Quality of Life Home Care Services, you’ll be a key leader in ensuring exceptional care and compliance for individuals receiving home and community-based services. You’ll supervise staff, guide service delivery, and maintain audit-ready documentation—all while shaping a culture of excellence and advocacy.
What You’ll Do
- Oversee and support direct care staff, ensuring services align with Coordinated Service and Support Plans
- Conduct periodic audits of case files and implement corrective actions
- Collaborate with support teams, consultants, and legal representatives to coordinate services
- Monitor outcomes and prepare reports for stakeholders
- Recommend service types and schedules based on client needs
- Uphold state and federal regulations with precision and professionalism
What We’re Looking For
- Proven leadership in human services, behavioral health, or social work
- Strong documentation and compliance skills
- Ability to manage teams and build relationships across disciplines
- Experience with individuals with developmental disabilities preferred
- Proficiency in program planning, evaluation, and stakeholder communication
Minimum Qualifications
You must meet one of the following:
- Bachelor’s in human services + 1 year full-time direct care experience
- Associate’s in human services + 2 years full-time direct care experience
- Diploma in human services + 3 years full-time direct care experience
- 50 hours of human services training + 4 years full-time direct care experience under qualified supervision
