Job Summary
The Direct Care Worker plays a vital role in providing compassionate and personalized care to individuals with various needs, including those with disabilities, chronic illnesses, or age-related challenges. This position involves assisting clients in their daily activities, ensuring their safety and well-being, and promoting their independence. Direct Care Workers are essential members of the healthcare team, working closely with clients and their families to implement care plans and support overall health.
Duties
- Assist clients with daily living activities such as bathing, dressing, grooming, and meal preparation.
- Implement individualized care plans tailored to each client's specific needs and preferences.
- Provide behavior management support for clients with autism or other developmental disabilities.
- Monitor and document client progress, reporting any changes in condition to the appropriate healthcare professionals.
- Assist in heavy lifting and transferring of clients as needed while ensuring safety protocols are followed.
- Support hospice care initiatives by providing comfort and companionship to clients in end-of-life situations.
- Maintain confidentiality by adhering to HIPAA regulations regarding client information.
- Collaborate with case management teams to ensure comprehensive care for clients in nursing homes or similar settings.
Skills
- Knowledge of care plans and ability to follow them accurately.
- Experience or training in case management practices.
- Understanding of nursing principles and practices is beneficial.
- Ability to perform heavy lifting safely and effectively.
- Proficiency in meal preparation that meets dietary restrictions or preferences of clients.
- Strong skills in behavior management techniques for individuals with autism or other behavioral challenges.
- Familiarity with hospice care practices is a plus.
