Job Description:
Join a world-class academic healthcare system, UChicago Medicine, as the Director, Community Health Initiatives responsible for leading community health initiatives for UChicago Medicine’s Urban Health Initiative (UHI).

The Director, Community Health Initiatives will create and implement innovative programs and evidence-based initiatives to advance health. This director will develop and lead Live Healthy Chicago for the UChicago Medicine (UCM) System and lead the implementation of the initiative for the South Side Accelerators for Chicago, including collaborating with the South Side Healthy Community Organization. The Director, Community Health Initiatives will also be responsible for planning engagement activities in collaboration with the UCM/UHI Offices of Community and External Affairs, and Health Transformation, UCM Community Advisory Councils, and the coordination and collaboration with community and civic stakeholders to develop and implement community programs. The Director will also cultivate and steward existing relationships with community members including government officials, not-for-profit organizations, social service agencies, healthcare providers, donors and the private business sector.

This position will report to the SVP, Community Health Transformation and contribute to achieving the goals outlined in UCM’s strategic implementation plans. Additionally, this position will work closely the Executive Director of Community and External Affairs and Executive Director of Community Benefits at UCM.

Note: This is a grant-funded position for one year.

Who you are:
A mission-driven community health leader with:
  • Bachelor’s degree; Master’s degree preferred
  • At least 10 years of experience in an impact-related project management role
  • Minimum of 8 years of experience in community affairs/community heath
  • Experience working in complex systems, including working across and partnering with multiple health systems
  • Experience with program implementation and evaluation
  • Valid Driver’s License and access to personal vehicle required
  • Excellent oral, written, presentation skills, and interpersonal communication skills with an ability to listen actively
  • Strong organizational skills, punctuality, self-motivation, aptitude for learning and working in diverse environments, and demonstrated computer (Microsoft Office) proficiency
  • Experience collaborating and coordinating with diverse stakeholders
  • Ability to exercise consistent discretion and judgement
  • Passion for and dedication to working with community members
What you’ll do as the Director, Community Health Initiatives:
  • Drive Live Healthy Chicago -South and related engagement activities.
  • Lead the execution of Live Healthy Chicago for UChicago Medicine and the south side. Oversee community programing and communications.
  • Oversee the collection of activities and track the impacts of the initiative.
  • Oversee and support community engagement and outreach activities.
  • Lead strategic initiatives and engagement activities for other programs, including community fitness and cancer outreach.
  • Align community health initiatives and partnerships.
  • Champion the strategy that aligns initiatives and partnerships to support the goal to improve the health outcomes with the diverse needs of the south side.
  • Build strong, rigorous analytical disciplines to measure and evaluate the tangible impact of Live Healthy Chicago and other programs.
  • Coordinate with Community Affairs and Care Coordination teams (patient advocate, community health worker and violence recovery programs).

Compensation & Benefits Overview

UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
