Position Details:
- Maintains caseload of 50+ families with minimum monthly contact.
- Acts as a liaison with Bucks County MH/DP, Behavioral Health, Children & Youth, referring agencies, families, child advocacy representatives/organizations, and stakeholder service providers.
- Assists in ensuring program compliance with all internal and State/County policies, regulations, and requirements.
- Complete intakes according to Merakey of Bucks County policies and procedures.
- Facilitating and participating in the assessment/evaluation, development, implementation, review of IFSP’s as required by program regulations and family needs.
- Assess child needs and family concerns.
- Secure pertinent information regarding child development.
- Coordinate implementation of intervention services.
- Educate families on outcomes and the family-training, routines-based model of intervention.
- Assisting the family in identifying available service providers and facilitating communication with and between the family and service provider.
- Participate in the Local Interagency Coordinating Council (LICC).
- Maintain all family mandated contacts.
- Maintain case management records in compliance with Merakey of Bucks County, Bucks County MH/DP, and the state of Pennsylvania policies and procedures.
- Refer appropriate clients to the BCIU #22 for transition.
- Attend and participate in transition planning meetings with the BCIU #22.
- Ensure all documentation is completed correctly, written professionally, and submitted within required deadlines.
- Input, update, and maintain all records in HCSIS Pelican system.
- Contact family, therapists, and agencies to schedule IFSP/team meetings.
- Obtain the medical necessity form from family’s physician if family does not comply with request.
- Research and inform families of appropriate community activities and resources.
- Attends internal and external meetings as required, including at least 24 hours of training per year.

QualificationsThe ideal candidate will possess the following:
- Bachelor's degree from accredited college or university
- One-year related experience
- Knowledge of child development preferred
- Valid Driver’s License and clean driving record
