Law Office of Shannon DeWall is searching for an Elder Care Coordinator. You must be motivated, have a keen attention to detail, and have a passion for working with seniors. This position requires meeting with clients in the office or their home for assessments as well as meeting with management staff at care facilities to build and maintain positive relationships. We are looking for someone who is comfortable working independently, proactive, possesses strong people skills, and is detail-oriented. You must have excellent communication skills, both verbal and written, and be compassionate and committed to working with and advocating for our elderly clients and their families. We want someone who is searching for a career and not just a job.
Responsibilities:
- Handling intakes for long-term care matters and serving as the client’s point of contact within the firm.
- Identifying the needs of our clients and connecting them with community resources and services such as home care, transportation, respite care, chore services, home technology and modification providers.
- Counseling client/family regarding aging issues.
- Providing client and family education, support and guidance on matters relating to client’s health and long-term care.
- Assisting with Medicaid applications.
- Intervening and advocating for client’s legal right to quality health care and long-term care.
- Scheduling quarterly visits with clients and their families.
- Providing community outreach and education (seminars, presentations, and workshops for the public).
- Serving as a spokesperson for the firm and developing relationships with key personnel in long-term care settings and other referral sources.
Qualifications:
- Degree in social work with an emphasis in geriatrics (or equivalent experience).
- Two years of experience in a long-term care setting or care management experience.
- Ability to problem solve and think quickly as circumstances change.
- Proficient with MS Office.
- Knowledge of Medicare, Medicaid and VA Benefits.
- Excellent written and verbal communication skills.
