PURPOSE OF THE POSITION
To support participants and families in receiving person-centered services at Aptiv. This may include but not be limited to: direct support and case management duties to individuals served on caseload, support and collaboration with departmental management team for service quality.
QUALIFICATIONS
PREFERRED
- Bachelor’s degree in a human services related field such as Sociology, Psychology, Communications, Anthropology, Education, Recreation Therapy, or other Bachelor’s Degree, Associates Degree or other post high school graduation coursework and three years experience in a human service field
- Experience working with children or adults with disabilities
- Well developed oral and written communication skills
- Experience in communicating rates for services and requesting authorizations
- Experience in case management and assessing developmental needs
- Knowledge of proactive behavioral approaches and precision teaching techniques
REQUIRED
- Valid Driver’s License
- Access to reliable non-public transportation with required insurance coverage for transportation for daily use
- Ability to operate computer equipment; knowledge of various software packages including Office Windows applications 2007 and Google applications
- Flexibility in planning work schedule
- Proficiency with time management and ability to meet deadlines
- Ability to provide direct support for individuals served within Employment
ESSENTIAL JOB FUNCTIONS
- Provide direct and indirect support to Aptiv participants in Employment Services
- Administer a person centered approach by assessing participant wants and needs and advocating for services that meet health, safety and independence needs of participants.
- Identify service gaps and collaborate internally and externally to initiate or expand programming consistent with the interests and support needs of the individuals served, their families/guardians and funders.
- Successfully complete case management related tasks in a timely manner. Activities include: prepare for and attend annual development plan meetings, maintain and update all necessary paperwork and database information, working closely with and accepting feedback from participants, families, funders, and various service providers.
- Complete and submit updates or reports as required by Aptiv and, or funding agencies. This may include participant updates and progress reviews, updates regarding services and updates to participant information as support needs evolve.
- Maintain professional relationships with families/guardians, funding sources, and additional support team members. Activities include: communicating effectively with individuals orally, in written form regarding participant’s services and have the ability to work in a team setting to promote cooperation, collaboration, and commitment.
- Communicate updates from caregivers and/or support team members to program leadership and/or staff when needed. This could include: changes in diagnosis, medications, living arrangements, general participant updates and concerns amongst other necessary communication. These updates are to be reported to appropriate team members in a timely manner.
- Serve as an advocate and assist individuals in understanding options for communicating their choices regarding services, living arrangements, work and other avenues of their lives supported by Aptiv. Advocate for these services with funders in a way that also meets the needs and goals of the funders.
- Encourage participants to build and utilize natural supports and become integrated into their community.
- Ensure accurate authorizations for Aptiv services are received initially and updated any time there is a change to programming. Complete at minimum, a bi-weekly review of authorizations to ensure accuracy. Provide oversight to ensure services are accurately scheduled in accordance to authorizations and that service codes and billing codes are correct.
- Maintain high performance standards. Activities include following organizational policies and procedures; attend team meetings; maintain job knowledge through training and orientation; provide quality customer service; manage time, use resources wisely, maintain solid working relationships, maintain compliance with annual training requirements and maintain confidentiality and ethical standards.
- Maintain current knowledge and assure compliance with applicable laws, rules, regulations, Aptiv policies and procedures, and state and national standards. Activities include periodic review of Aptiv policies and procedures and state and national standards; and maintain and practice all aspects of safety while performing duties.
- Explore areas of growth and develop new skills from additional training outside of Aptiv.
- Work in collaboration with the department team which may include the manager, lead,and behavior specialist.
EXPECTED OUTCOMES
- Ensure 90% of all completed case load file reviews to be up to date
- Complete 5-7 hours of direct support weekly
- Ensure all participants on assigned caseload have updated all necessary information and paperwork on company database (SETWorks)
- Ensure authorizations requested are accurate.
- Ensure hours served don’t exceed authorized units
- Communicate needs to adjust service hours, ending of services, and promote new services
PHYSICAL/SENSORY/COGNITIVE REQUIREMENTS
The work time will be spent in the office and in programming spaces and in the community settings. Occasional travel required. Mobility within the rural and urban community is required on a weekly basis that may or may not be accessible. Occasionally may push/pull wheelchairs and assist with transfers in or out of wheelchairs for adults and youth of various sizes. Regularly required to sit at a desk or in meetings for an extended period of time. Ability to effectively present information and respond to questions from participants, staff, customers, vendors, and the general public when communicating using technology or in face-to-face interactions.
Lifting weights of up to 20 pounds regularly and up to 50 pounds occasionally. Lifting requirements may change according to schedule or department.
Uses hearing and vision with the ability to collaborate and orchestrate in a variety of environments. Regular use of fine motor skills for writing or typing.
Regularly will concentrate on multiple tasks simultaneously with the capacity to respond calmly and quickly to emergency situations. Ability to interpret an extensive variety of technical instructions and handle several abstract and concrete variables. Ability to remember and/or track multiple verbal and written tasks/assignments. Capacity to differentiate between various levels of confidentiality and act accordingly.
ENVIRONMENTAL CONDITIONS
Work is performed in both the office setting and within the community with varied conditions. Outdoor travel is required occasionally. Staff are expected to work in an environment with participants who can be verbally and/or physically aggressive to staff, other participants, community members, and themselves. Aptiv provides training and support to prevent, de-escalate and manage these incidents, but cannot guarantee complete absence of such occurrences.
