This full-time position is based within the bustling and supportive settings of Cuyahoga County, specifically focusing on the Cleveland Eastside. Duties and Responsibilities
- Develop and implement comprehensive care plans tailored to the specific needs of each patient.
- Collaborate with physicians, social workers, chaplains, and other professional staff to ensure the coordination of services.
- Provide direct nursing care by evaluating patient health problems and needs.
- Administer nursing care to ill, injured, convalescent, or disabled patients.
- Advice patients on health maintenance and disease prevention or provide case management.
- Supervise and monitor nursing activities of assigned hospice aides.
- Facilitate family meetings to discuss end-of-life care preferences and plan accordingly.
- Document patient's condition and nursing needs accurately and in a timely manner.
- Adjust care plans as necessary to ensure optimal patient outcomes.
- Communicate with family members and provide updates and education on patient care and management.
- Manage pain and other complex symptoms effectively, enhancing comfort for patients.
- Lead initiatives for improving the quality of care and patient satisfaction.
Requirements
- Current Registered Nurse (RN) license in the state of operation.
- Minimum of three years of experience in clinical nursing, with at least two years in a hospice or palliative care setting.
- Demonstrated leadership potential, with abilities in team management and decision-making.
- Exceptional bedside manner with a proven track record in empathetic patient care.
- Advanced skills in pain and symptom management, with a deep understanding of palliative care principles.
- Effective communication skills, including both verbal and written proficiency.
- Strong organizational abilities and a high level of attention to detail.
- Commitment to the ethical guidelines of the nursing profession, particularly in a hospice environment.
- Ability to work under pressure in a fast-paced setting.
- Mandatory participation in continuous education and professional development.
- Flexibility in working hours, as patient needs can often extend beyond standard working hours.
- Membership or eligibility for the ONA Union.
