Cabin Creek Health Systems (CCHS) is looking for a Family Nurse Practitioner to join our community health center network. This position is located within our health clinic. We offer a flexible schedule and excellent fringe benefits.
The FNP addresses primary care, acute care, and infectious disease in populations among a diverse population within the area.
Cabin Creek Health Systems (CCHS) is a Federally Qualified Health Center (FQHC), Patient Centered Medical Home, with six integrated health centers, and five School Based Health centers, and is continually planning expansions and new program developments. CCHS offers many services including primary care, dentistry, 340B pharmacy programs, behavioral health, robust substance use treatment, pulmonary rehab, and many more.
Responsibilities and Skills Required
- Primary Care Provider providing complete patient history, physical examinations, diagnosis, and plan for treatment of primary, secondary, and tertiary care services as outlined in, and in compliance with the standardized protocols for Nurse Practitioners. Primary care delivery includes communicating with patient or guardian of patient regarding care plans, results of diagnostic studies, and answering patient and family questions. Also included is organizing and delivering health education for patients, including general health information and disease specific education. The primary care provider coordinates patient referrals for specialized health care to public health care agencies.
- Establish and maintain a confidential medical record for each patient.
- Organize and administrate delivery of services offered by CCHS.
- Communicate with Chief Medical Officer and regarding quality care and treatment issues of patients at CCHS.
- Assure that services are provided in accordance with specified policies of CCHS and regulatory agency's policies.
- Serves as a resource and collaborative partner for community agencies.
- Ability to effectively use an electronic health record.
- Ability to compassionately care for complex patients.
- Ability to maintain calm under pressure and to prioritize patient centered care.
- Current WV APRN Licensure

CCHS is a Federally Qualified Healthcare Center (FQHC). Nurse practitioners may be eligible for school loan repayment under the National Health Service Corps (NHSC) loan repayment programs in conjunction with the Health Resources Services Administration (HRSA).
