JOB SUMMARY
The Health Coach at MH² plays a key role in helping patients implement sustainable lifestyle changes that support both metabolic and mental health. This role involves individualized coaching, team collaboration, and educational programming across multiple domains of lifestyle medicine—nutrition, exercise, sleep, stress, and substance use. The ability to support patients following a therapeutic ketogenic diet is required, but this role also requires flexibility in supporting a range of dietary patterns and patient preferences.
Coaching is provided to individuals with serious mental illness, co-occurring metabolic conditions, or complex chronic health challenges. Care may take place in our Wellesley clinic, virtually via telehealth, or in patients’ homes and community settings, depending on patient needs and program assignment.
ESSENTIAL FUNCTIONS
- Deliver one-on-one and group-based health coaching focused on diet, exercise, sleep, stress management, and behavior change
- Support patients in initiating and sustaining ketogenic and other nutrition therapies, in close collaboration with dietitians and medical providers
- Help patients identify goals, track progress, and troubleshoot barriers using motivational interviewing and other evidence-based methods
- Provide in-home or community-based coaching and support for select patients, including those enrolled in higher-touch or recovery-focused programs
- Co-lead wellness education groups and community-building activities that foster support, literacy, and hope
- Collaborate with psychiatrists, therapists, dietitians, and health professionals in care planning and team meetings
- Document coaching sessions and care plans clearly and consistently in the electronic health record (EHR)
- Participate in staff meetings, training, and ongoing professional development
PHYSICAL DEMANDS / WORKING CONDITIONS
- Hybrid role including in-clinic care, virtual sessions, and in-home/community visits
- Must be able to travel locally for home or community-based visits (valid driver’s license and reliable transportation required; mileage reimbursed)
- Must be able to carry lightweight clinical materials (e.g., scale, tablet, printed resources)
- Ability to sit, stand, and use a computer for prolonged periods
QUALIFICATIONS
Education
- Required: Bachelor’s degree in a health-related field (e.g., nutrition, exercise science, psychology, public health)
Licensure / Certification
- Required: Health and Wellness Coach Certification from a nationally recognized program (e.g., NBC-HWC, ACE, Wellcoaches)
- Preferred: Certification in exercise physiology, fitness, or nutrition
- CPR required within 3 months of hire
Experience
- Required: Minimum 2 years of experience in health coaching or behavior change counseling
- Preferred: Experience working with individuals with serious mental illness or complex chronic conditions
- Preferred: Experience supporting ketogenic or therapeutic diets, or willingness to learn
Knowledge, Skills, and Abilities
- Demonstrated ability to coach in a compassionate, nonjudgmental, and effective way
- Familiarity with lifestyle medicine, chronic disease management, and mental health care
- Comfortable working with clients in one-on-one, group, and virtual settings
- Excellent communication and interpersonal skills
- Proficiency with EHR systems, telehealth platforms, and basic clinical documentation
- Commitment to interdisciplinary, collaborative care
Salary:
Full-time: $60,000 - $75,000 annually, depending upon experience and qualifications
