As a part of the care team you will perform a variety of duties that relate to client care including care consultations with potential clients and family members, general office activities, and establishing relationships with potential clients and referral sources.

Position Requirements:
- Scheduling and hiring team members
- On-call support as part of the on-call rotation
- Identify and establish relationships with potential clients and referral sources
- Coordinate with internal teams for client care coordination
- Work with clients providing care
Qualifications:
- Healthcare or home care experience
- Strong communication and interpersonal skills
- Results-oriented and self-motivated
