Key Responsibilities
- Provide management and delivery of individualized patient care plans
- Collaborate with the interdisciplinary care team to ensure patient needs are met
- Deliver high-quality patient care in compliance with clinical standards
- Complete documentation accurately and meet required charting deadlines
- Manage time, caseload, and schedule independently
- Utilize EMR systems, tablets, and mobile devices to support patient care
Required Qualifications
- Associate degree or higher in Nursing
- Valid Registered Nurse (RN) license in the state of Pennsylvania
- At least 1 year of nursing experience (med/surg or similar)
- Valid driver’s license with ability to travel 20–25 miles around Greensburg, PA
- Experience with Electronic Medical Record (EMR) systems (any)
- Current certification(s) in one or more of the following:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Preferred Qualifications
- Prior home health or hospice experience
- At least 1 year of med/surg nursing experience
- RN license number provided on application
