Position Summary:
The Market Liaison (or Community Liaison) serves as the primary point of contact between the
organization and its referral sources, community partners, and patients. This role focuses on
building and maintaining strong relationships to support patient referrals, community
engagement, and awareness of home health and hospice services. The Market Liaison works
closely with clinical and administrative teams to ensure seamless coordination of care and
growth of the organization’s programs.
Key Responsibilities:
- Develop, maintain, and strengthen relationships with community partners, including
- hospitals, clinics, physician offices, skilled nursing facilities, and senior living
- communities.
- Conduct in-person visits, educational presentations, and follow-ups to build awareness
- of services.
- Collaborate with intake and clinical teams to coordinate referrals and patient
- transitions.
- Represent the organization at community events, health fairs, and professional
- meetings.
- Maintain detailed records of community interactions, referrals, and outcomes in the
- CRM system.
- Assist in implementing strategic marketing and outreach plans to achieve organizational
- goals.
- Provide feedback to leadership on market trends, referral opportunities, and
- competitive insights.
Qualifications:
Minimum 1–2 years of healthcare sales, business development, or community liaison
experience (home health/hospice preferred).
