Position: Intake Specialist
Department/Program: Care Coordination Program (CCP # 400)
STATUS: Full-time Part-time Exempt Non-exempt
Reports to: Care Coordinator Supervisor
Solutions for Care (SFC) serves over 12,000 older adults and those living with disabilities each year in Berwyn, Cicero and Proviso Townships. We provide direct client service through information and assistance, in-home care, advocacy services and transitional assistance in hospitals. General Statement of Duties:
Under the leadership of the Care Coordinator Supervisor, the Intake Specialist will work with the Care Coordinator staff who are responsible for programs and services under the State of Illinois Community Care Program. The Intake Specialist is responsible for the initial screening of individuals, providing information and referrals to callers, and a variety of administrative tasks. The Intake Specialist will maintain a professional manner during all interactions with the public while on the phone or in person. The Intake Specialist will maintain current information regarding opportunities and services available within our service areas which include Berwyn, Cicero, and the Proviso Township area.
Qualifications:
Bilingual: Must be able to read, write, and speak in English/Spanish. One year of experience working with older adults or disabled individuals is preferred. Must demonstrate strong organizational and time management skills and be adaptable to changing work conditions. Strong communication skills, computer literacy and the ability to work with diverse populations is required. Must display appropriate telephone etiquette during all phone call handling.
Education and other Requirements:
Minimally a High School Diploma or GED is required.
A background check is required.
Proficient in Microsoft Office Suite, including Outlook, Word, Excel and Adobe.
Training:
On site, paid training will be provided.
Duties and Responsibilities:
- Develop an understanding of programs, benefits, and services available to seniors and adults with disabilities on a local, regional, state and federal level.
- Gather and accurately document relevant information from potential clients and their family members to make first level assessment of need and eligibility utilizing the IDOA Intake form.

Assess client needs in a sensitive and professional manner and assist with applications.
- Connect clients to appropriate resources..
- Provide follow-up to ensure needs are me.
- Ensure that billing, coding, and case assignment timeframes are met for all referrals.
- Receive case transfer requests and ensure they are processed according to timeframes.
- Meet regularly with supervisors to discuss priorities or other issues prior to case assignments.
- Display appropriate telephone etiquette during all phone call handling.
- Maintain a professional and confidential manner in all interactions with the public .

- Assist in completing required reports to funders as needed.
- Maintain accurate case note entries, documentation, reports, and record keeping.
- Complete all required forms in a timely manner.
- Will occasionally cover for the receptionist as needed.
- Assist with other administrative duties as needed
- Other duties as assigned by supervisor and senior leadership staff.

- Must be able to sit and work at workstation for extended periods of time.
- Must be able to walk, climb stairs, use elevators, and conduct visits in all kinds of weather, if
requested by supervisor.
Schedule:
Full time: 8:30 am to 4:30 pm, Monday thru Friday.
Salary:
Salary Range: $35,500.00 to $38,000. During the first year, six-month and 12-month performance reviews are conducted. Thereafter, performance reviews will be conducted every 12 months. Salary increases are based on performance.
