Job Summary
We are seeking a dedicated and compassionate Field Nurse to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in their homes or community settings. This role requires a strong commitment to patient care, effective communication skills, and the ability to work independently. The Field Nurse will be responsible for assessing patient needs, developing care plans, and coordinating with other healthcare professionals to ensure optimal patient outcomes.
Responsibilities
- Conduct comprehensive home care assessments and physical examinations of patients.
- Develop, implement, and evaluate individualized care plans based on patient needs and preferences.
- Provide nursing care in various settings including long-term care facilities, hospice care, and critical care environments.
- Perform triage assessments to prioritize patient needs effectively.
- Administer medications and treatments as prescribed while monitoring for side effects or complications.
- Educate patients and families on health management, disease prevention, and self-care techniques.
- Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
- Maintain accurate documentation of patient progress and interventions in compliance with healthcare regulations.
Skills
- Proficiency in home care practices and procedures.
- Strong knowledge of physical examinations and basic life support techniques.
- Experience in long-term care, ICU, triage, case management, and hospice care is highly desirable.
- Familiarity with teaching hospitals and critical care environments is a plus.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Strong organizational skills with the ability to manage multiple cases simultaneously.
- Compassionate approach to patient care with a focus on empathy and respect for individual needs.
