Job Description
The Maternal and Child Health Coordinator (MCHC) has a wide array of responsibilities that are designed to ensure the overall quality of care for the pregnant patient (prenatal and postpartum) and her newborn child. These responsibilities include tracking, case management, and education of the patient from initial pregnancy diagnosis, throughout prenatal care and delivery, as well as postpartum follow-up. The MCHC also functions as a member of the patient care team, performing medical assistant duties. The MCHC approaches his/her tasks in a team-based care fashion that supports patients and their families in self-management, self-efficacy, and behavior change.
Please note: This position is located at the Commerce City, CO clinic only and is Not Remote.
We offer:
- $21.50 to $28.00 per hr.
- Monday to Friday – 8 am to 5 pm

Specific Duties:
Works in conjunction with other Medical Providers, Dental Providers, Behavioral Health Providers and all other staff to ensure that the patients obtain the highest standards of integrated healthcare
- Acts as a patient advocate for our vulnerable populations by ensuring that patients are fully informed and comfortable with their visit to SALUD
- Follows all procedures for the electronic health records system
- Documents patient information accurately in accordance with established quality measures and standard procedures i.e. vital signs, reason for visit, medications administered
- Assures patients are notified of laboratory, imaging, and other information as directed by the provider within established timeframes and appropriately documented in the patient record
- Adheres to all HIPAA standards and protocols surrounding patient confidentiality
- Maintain patient flow throughout the clinic by utilizing a proactive approach and anticipating the necessary supplies and setup for each patient visit
- Provide continuity of care to pregnant patients
- Keeps OB data up to date and meets with hospitals as needed
- Works with OB team (Provider, Medical Support Staff, Patient Health Advocate, Behavioral Health, to foster an environment conducive for the monitoring and overall health of all OB patients
- Provides advice for OB patients calling with acute problems
- Provides Resource Information and collaborates with community resources
- Triage Patient Calls and coordinate referrals
Provides different levels/types of education to patients, including General Medical Assistant Duties include, but are not limited to, the following:
- Assists with numerous medical, surgical, and gynecological procedures
- Performs autoclave sterilization of instruments
- Adheres to infection control policies and procedures
- Collects, processes, records, and educates patients on a wide number of medical laboratory tests
- Administers and maintains medications at the highest standards of patient care as per Salud Standard Operating Procedures
- Stores, inventories, and administers immunizations in compliance with all state and federal guidelines
- Performs EKGs, Spirometry, vision acuity, ear irrigation, and other tests/procedures as directed by the provider
- Assesses and schedules patients accurately by phone and in person
- Responds appropriately to medical emergencies
- Career ladder positions at Salud are required to complete the career ladder that is most closely associated with their role. This includes completing all the courses, passing the exams, having satisfactory reviews, and obtaining the outside certification as quickly as these levels are available and within the parameters established by Salud. Those unable to complete the ladder within the parameters set by the organization will not be allowed to continue in their current role
- Maintains basic CPR certification at all times
- May be required to travel to other clinics based on staffing shortages
- May be assigned to work virtually
- May perform interpretation duties if they hold the necessary qualifications
- Performs other duties as assigned
Qualifications
Education:
- High school diploma or equivalent required
- All Medical Support Associates must be certified within 6 months of hire, and certification must be maintained throughout employment
- Medical Assistants, without any formal Medical Assistant training, are required to have a current Certification/Registration as a Medical Assistant
Experience:
- Previous healthcare experience preferred
- History of working with an OB population is preferred
Licensure:
- Must maintain Salud credentialing at all times
Knowledge, Skills, and Abilities:
- Bilingual English/Spanish preferred
