We are seeking a detail-oriented and compassionate MDS Coordinator to join our dedicated team of healthcare professionals. This pivotal role ensures that our residents receive the highest quality of care while maintaining compliance with state and federal regulations and maximizing appropriate reimbursement for services.
Position Summary:
The MDS Coordinator oversees the resident assessment process, ensuring timely and accurate documentation. This role plays a key part in developing individualized care plans and works closely with nursing staff, therapists, physicians, and families to promote effective, person-centered care. The MDS Coordinator also ensures that all assessments meet regulatory guidelines and support the financial integrity of the facility.
Key Responsibilities:
- Assessment & Documentation:
Complete and coordinate comprehensive resident assessments using the MDS (Minimum Data Set) and related tools. Ensure timely, accurate, and thorough documentation.
- Care Planning:
Collaborate with the interdisciplinary team to develop, implement, and revise individualized care plans based on residents’ evolving needs and preferences.
- Compliance & Reimbursement:
Maintain compliance with federal and state regulations regarding assessments and care planning. Optimize reimbursement through accurate coding and timely submissions.
- Communication & Coordination:
Serve as a liaison between care team members, residents, and families. Facilitate clear communication about care goals, progress, and changes in condition.
- Quality Assurance:
Monitor resident care outcomes, participate in quality improvement initiatives, and identify opportunities to enhance resident care and operational efficiency.
Qualifications:
- Current RN license (state-specific) required
- Experience with MDS 3.0 assessments and care planning preferred
- Strong understanding of Medicare/Medicaid regulations and reimbursement
- Excellent attention to detail, organization, and communication skills
- Ability to work collaboratively with a multidisciplinary team
