We are looking for an experienced MDS coordinator to join our team.
- Completes assessments, Minimum Data Set (MDS), and care plans for all residents assigned.
- Monitors the completion of MDSs by other disciplines within time frames prescribed by regulatory guidelines.
- Advises supervisor of incomplete and untimely assessments by disciplines other than nursing.
- Initiates care plans and supporting activities that will result in the best possible outcome for assigned residents.
- Maintains and updates all care plans and assessments for assigned residents every quarter (at minimum) and adds/deletes issues as necessary.
- Monitors and ensures completion of documentation by nursing staff within prescribed MDS time frame.
- Monitors and audits clinical record documentation on assigned residents to ensure accuracy and timeliness.
- Promotes and participates in ongoing education, inside and outside the facility, to maintain and enhance knowledge of current/new regulations.
- Maintains current knowledge MDS assessment process.
Job Type: Full-time
Pay: $32.00 - $35.00 per hour
