Position Title: Telephonic Medical Assistant – Chronic Disease Care Coordination
Location: Remote
Job Type: Full-Time 8:00am-5:00pm with an hour lunch (Eastern Time Zone)
Overview: We are seeking a reliable and trustworthy Medical Assistant to join our healthcare team in providing comprehensive care coordination for geriatric patients with chronic diseases. The successful candidate will be key in supporting patients and providers, ensuring quality care, and promoting positive health outcomes through effective communication and documentation.
Key Responsibilities:
- Chronic Disease Management Support: Assist in the coordination and management of chronic diseases (e.g., diabetes, hypertension, COPD, heart disease) for a geriatric patient population.
- Care Coordination: Serve as a point of contact between patients, families, and the healthcare team to ensure seamless care delivery. Manage follow-up care and track treatment care plans.
- Patient Education: Provide education to patients and families on chronic disease management, medications, lifestyle changes, and preventive care strategies.
- Electronic Health Record (EHR) Management: Accurately document patient information, treatment plans, care coordination efforts, and any relevant notes in the EHR system.
- Medication Management: Assist with medication refills, tracking patient compliance, and educating patients on proper medication usage.
- Interdisciplinary Collaboration: Work closely with physicians, nurses, care managers, and other healthcare professionals to develop and implement individualized care plans for each patient.
- Patient Advocacy: Ensure that patients' needs and concerns are addressed in a timely and compassionate manner, fostering trust and promoting overall well-being.
- Administrative Support: Assist with scheduling, making phone calls, managing patient records, and performing other clerical tasks as needed.
Qualifications:
- Education: High school diploma or equivalent required; Medical Assistant certification preferred (e.g., CMA, RMA).
- Experience: Minimum of 1-2 years of experience in a healthcare setting, preferably in geriatrics, chronic disease management, or care coordination. Have a background in endocrinology. Have experience with Blood Glucose Monitoring
- Skills:
- (Preferred) Previous experience working telephonically
- Strong knowledge of chronic diseases common in the geriatric population (e.g., diabetes, hypertension, dementia, etc.).
- Proficiency in using Electronic Health Records (EHR) systems for documentation.
- Excellent communication skills, with the ability to interact professionally with elderly patients and their families.
- Ability to prioritize tasks and manage time effectively in a fast-paced environment.
- Strong organizational skills and attention to detail.
- Compassionate, empathetic, and patient-centered approach to care.
Working Conditions:
- Desktop with Telephonic patient interaction.
- Monday through Friday 8:00am-5:00pm with an hour lunch work schedule.
