Forum Health is a nationwide network of medical providers specializing in precision integrative and functional medicine. We are seeking a reliable, compassionate, and detail-oriented Medical Assistant / Front Desk Receptionist / Therapy Tech to support both front desk operations and clinical care in our fast-paced, patient-focused Brookfield and Mequon offices.
This 32-40 hour hybrid role blends medical assistant duties, front desk reception and therapy support.
Position Overview:

Responsibilities include greeting patients, managing administrative tasks, assisting with phlebotomy and IV therapy, and supporting clinical documentation and procedures. This is a unique opportunity for someone eager to grow within a holistic and innovative medical setting.
Key Responsibilities:
Administrative & Reception Duties
Greet and check in patients with professionalism and warmth
Answer phones, emails, and faxes; relay accurate messages
Schedule and confirm appointments, coordinate follow-ups
Process invoicing and inventory management
Ensure smooth patient check-in and check-out, including payment processing
Maintain an organized front desk and patient-facing environment
Clinical & Medical Assistant Duties
Collect vitals and other screening; complete medication/supplement/allergy reconciliation
Assist patients with therapies such as entering and exiting the soft-sided hyperbaric chamber and running equipment (training provided)
Assist in procedures if needed
Update EMR documentation accurately
Coordinate lab testing, medication refills, and pharmacy communications
Manage medical records, faxes, and prior authorizations
Set up and maintain medical equipment
Qualifications:
High School Diploma or GED required
Phlebotomy experience required
1–2 years of experience in a medical office, integrative/functional medicine preferred
Familiarity with EMR systems and Microsoft Outlook 365
Strong multitasking, organizational, and communication skills
Professional appearance and demeanor; team-oriented mindset
Applicant must be authorized to work in the United States without the need for sponsorship now or in the future
Schedule & Work Expectations:
Schedule: Approximately 32–40 hours per week
Typical Hours: 6-8 hours per day, Monday through Thursday; some Fridays may be required based on clinic needs from time to time (i.e. staff out on planned PTO)
Work Location: On-site at both Waukesha (1930 W Bluemound Rd Suite C) and Mequon (10602 N Port Washington Rd Suite 101)
