Medical Social Services position:
All Heart Home Care is seeking a qualified Licensed Medical Social Worker for PRN work for our clients in the Middle Tennessee area. Services include advising family and care givers, providing patient education and counseling, and making referrals for other services. The Social Services Program also assists patients and their families with personal and environmental difficulties that interfere with obtaining maximum benefits from medical care. This position does involve direct patient visits.
QUALIFICATIONS:
- Master's Degree Social Work
- Current, valid, and unencumbered Tennessee Licensed Master Social Worker or Licensed Clinical Social Worker
- Minimum two years of professional experience as a MSW or LCSW
- 1 year of recent Home Health experience preferred
- Current and valid driver license, transportation, and auto insurance
- Working knowledge of Medicare and OASIS assessments
- Geriatric Care Management experience preferred
