What You'll Do:
- Provide in-home or on-site care at private residences, senior living communities, and long-term care settings
- Manage chronic conditions, preventive care, and mental health needs for an aging population
- Collaborate with interdisciplinary teams to deliver whole-person, senior-focused care
- Build trusted relationships through empathy, consistency, and patient education
- Support care access and equity by removing barriers for vulnerable seniors
- Must have valid Driver's License and reliable transportation
