Job Overview
We are seeking a compassionate and dedicated Hospice Nurse Practitioner to join our team. In this role, you will provide comprehensive face to face visits for patients with life-limiting illnesses, focusing on enhancing their quality of life and supporting their families. You will work collaboratively with a multidisciplinary team to develop and implement patient-centered care plans, ensuring that all aspects of patient needs are addressed.
Duties
- Conduct thorough assessments of patients’ physical, emotional, and psychosocial needs.
- Provide education and support to patients and families regarding disease processes, treatment options, and end-of-life care.
- Manage symptoms effectively through appropriate interventions, including psychopharmacology when necessary.
- Maintain accurate documentation of patient assessments, interventions, and outcomes in compliance with regulatory standards.
Requirements
- Current Nurse Practitioner license with certification in hospice or palliative care preferred.
- Certification in Basic Life Support (BLS) is required.
- Excellent communication skills to effectively interact with patients, families, and healthcare professionals.
- Ability to work collaboratively within a multidisciplinary team environment.
- Strong organizational skills and attention to detail in managing patient records.

Pay: $70.00 - $72.66 per hour
Expected hours: 5 – 10 per week
