Summary
We are seeking an PT/OT to join our team at 1st Americare, LLC. In this role, you will provide essential therapeutic services to clients in the comfort of their homes, helping them achieve their personal goals and improve their daily living skills. Your expertise will be vital in enhancing the quality of life for our clients.
Responsibilities
- Conduct comprehensive assessments of clients’ needs and develop individualized treatment plans.
- Implement therapeutic interventions to improve clients’ physical and cognitive abilities.
- Collaborate with other healthcare professionals to ensure coordinated care.
- Educate clients and their families on treatment plans and strategies for success.
- Monitor client progress and adjust treatment plans as necessary.
- Maintain accurate documentation of client assessments and progress notes.
Requirements
- Master's degree in OT/PT from an accredited program.
- Current state licensure as an PT/OT.
- Strong knowledge of home health care practices and regulations.
- Excellent communication and interpersonal skills to build rapport with clients and families.
- Ability to work independently while managing a caseload effectively.
