Come work with an adult male in his home, with your care and compassion, you can assist him in Deer Park, WI
The schedule is part-time. The schedule is flexible 4-8 hours per week with potentially extra on the weekend.
Pay rate: $17.00-$19.00 per hour and $.35 per mile for transportation with member.
Tasks include but are not limited to:
Potentially some transportation.
Companionship
Make meals and assist with feeding as needed.
Ambulation assistance
Assistance to and from the restroom

Looking for someone with some experience preferred, willing to learn, conversational/talkative, have upper body strength, and a calm personality. Willing to train the right person. A driver’s license, dependable transportation, and a favorable background are important.
Who is Community Horizons
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
