We are looking for a Full or Part Time RN for the Wentzville/Warrenton area.
Home Health RNs:
Manage a caseload of patients, working with the patient, their family, and their physicians to determine how often and when to make skilled nursing visits;
Coordinate with patients and physicians to develop a plan of care;
Provide nursing care, education, and treatments as ordered;
Work with PT, OT, ST, and Social Work to provide for patients’ needs; and
Perform and complete Start of Care, Recertification, Resumption of Care, and Discharge Comprehensive OASIS assessments.
