Administrator of the program for the lovely 29 year old patient is on site daily to support the team. She has over 29 years of experience dealing with complex patients. Summary
We are seeking a Registered Nurse to join our team at GEE HCS CDS. This role is crucial in delivering exceptional patient care and supporting our commitment to health and wellness as outlined in the Texas HCS plan. The ideal candidate will be responsible for assessing patient needs, helping develop care plans, and providing direct nursing services as outlined by the HCS program.
Responsibilities
- Conduct comprehensive assessments of patients' health status.
- Help develop and implement individualized care plan based on patient’s needs.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' progress and adjust care plan as necessary.
- Educate patient and her family about health conditions and any new treatment options.
- Collaborate with healthcare team members to ensure coordinated care.
- Show respect for other team members and family while focusing on not just physical care, but also remembering the social, and emotional support of the sweet young lady.
- Maintain accurate documentation of patient interactions and interventions as outline by Texas HCS.
- As needed, RN will accompany patient, along with at least one family member to appropriate outings and medical care in the community.
- Work with team-based care, since a caregiver will often be present to help the patient support care in the home.
Requirements
- Current Registered Nurse (RN) license in the state of Texas.
- Experience with complex patients is helpful.
- Strong knowledge of aseptic techniques and pain management practices.
- Ability to work effectively with family utilizing existing set up of care established over the past 29 years of nurses and caregivers.
- Excellent communication skills, both verbal and written.
