Summary:
The RN - Hospice Admission Development Coordinator is responsible for coordinating and/or conducting initial patient/family consultation and admission. This individual may be the first contact between Hospice and potential Hospice patients. This position requires extensive knowledge of Hospice rules and regulations, the ability to share Hospice benefits with potential patients, and ensure admission to Hospice proceeds in a timely fashion and is designed to meet the patient’s needs.
The Coordinator is responsible for ensuring that standards of care and policies related to admission are consistently followed and patient, family, and physician satisfaction is a priority. The Coordinator works closely with the Hospice team to ensure good communication, coordination of care, and successful admission of patients choosing Hospice Care. As an integral part of the Hospice team, this individual cooperates with other departments to assist with Ballad philosophy, to improve quality of Hospice service, and reduce costs. The Coordinator maintains harmonious interactions between departmental personnel and their customers, including patients, family members, visitors, physicians, clergy, other system team members, and the general public. The Coordinator takes every opportunity to promote Hospice and educate the community about Hospice care.
Requirements:
Associate's or Bachelor's degree from an accredited Nursing program required. At least one year RN experience required. Must have a good knowledge of hospice and successfully complete agency orientation. Must have dependable transportation. Must carry current personal collision and liability insurance on any vehicle driven. Must maintain current driver’s license with the appropriate state. Must possess and maintain current certification in cardiopulmonary resuscitation.
The competency and practice of point of care testing is under the direct supervision of the Laboratory Medical Director and/or designee per regulatory rule #1200-6-3-15 Alternate Site Testing, Section (1)(c), 9 of the Tennessee Medical Laboratory Board Division of Health Related Boards. This practice is a collaborative process with the Laboratory and Nursing leaderships.
