Beautiful mid-sized long term care facility on the Upper East Side is seeking a full time MDS Assessor Monday - Friday.
RN must have long term care + MDS experience
Job responsibilities include but are not limited to the following:
- Completing accurate assessments, MDS & care plans as assigned.
- Initiating care plans and supporting activities as assigned.
- Monitors MDS and care planning documentation for all residents; ensures documentation is present in the medical record to support MDS coding.
- Monitoring & auditing clinical records, ensuring accuracy & timeliness
- Informing Director Of Nursing of persistent issues related to non-compliant documentation.
- Protecting the confidentiality of Resident & Facility information at all times
Requirements:
- New York State Registered Nurse license
- Long-term care experience required
- Must be highly organized, professional & motivated
- Should have solid computer skills
- Excellent communication skills
Job Type: Full-time
Pay: $95,000.00 - $105,000.00 per year
