Active Solutions Inc. is currently seeking an RN that will be a great fit with our ASI family and who is able to help support individuals and families who receive Developmentally Disabled Waiver Services in our Las Cruces office. Active Solutions Inc. will provide all training, mentoring and supplies that are needed so that we can support you and enable you to do your best!! The RN will work hand in hand with our Las Cruces Team that consists of a Program Manager, Service Coordinators, Administrative Assistant, another experienced RN, as well as the Moxie Team that consists of Direct Support Staff and IDT members.
The nurse will be responsible to provide and document nursing services that meet the requirements of the DD Waiver Standards to assure quality health care for the individuals we serve. The nurse will work with Individuals, Family Members, Guardians, Service Coordinators, Case Managers, Surrogate Providers and other DDW Agencies to determine the individual’s level of need for nursing services.
Duties Include:
- Conduct face to face visits per assessment acuity levels
- Input individual’s information into Therap database
- Provide trainings to Direct Support Providers on Individual Health Care Plans and CARMPs
- Complete Individual Annual Nurse Assessments and on an as needed basis due to changes in medical conditions of the individual served
- Complete Semi-Annual and Annual Nursing Reports
- Create Health Care Plans as needed
- Attend Annual Individual Service Plan (ISP) meetings and Interdisciplinary Team (IDT) meetings as scheduled
- Review Health Care Plans every six months
- Review Medical Consultation forms for all individuals served by Active Solutions Inc.
- Must be able to work with IDT Members (CM, SC, Therapists, PCP, etc.), Las Cruces Team (Program Manager, Administrative Assistant, RN, SCs, DSPs) and Albuquerque Office (President, Operations Manager, etc.)
- Be available by phone and text message
- Respond to phone calls, emails and text messages within 24 hours
- Communicate on a daily basis with Program Manager, Service Coordinators and be available for calls and consultation as needed
- Model professional behavior and communication when representing Active Solutions Inc
- Maintain current BLS Certification, AWMD, ANE and other required State Trainings (ASI will help you to keep trainings up to date)

Qualifications:
- Current New Mexico RN License
- Reliable Vehicle
- Current Car Insurance and Registration
- Experience working with individuals with intellectual and developmental disabilities preferred but not required
- Able to pass a background check
