This position provides primary coverage for adult cardiac patients, offering comprehensive psychosocial support to a chronically ill and vulnerable population. The social worker conducts thorough assessments, offers crisis intervention, and provides counseling to patients and their families. Responsibilities include coordinating complex care needs, developing and implementing detailed discharge plans to acute and subacute rehabilitation facilities, and connecting patients with community resources and support services. The role involves close collaboration with the multidisciplinary medical team to ensure optimal care and support for adult patients with cardiac conditions, including heart failure and post-operative care.

This is a day shift, Monday - Friday 9am-5pm position with rotating weekend/holiday coverage.

Preferred Criteria

• At least 2 years of In-Patient experience

• Cardiac Care Discharge Planning experience

Required Criteria

• Master’s Degree in Social Work (MSW)

• New York State License as a Licensed Masters Social Worker (LMSW)
