RSS is seeking a full-time Care Manager/Service Coordinator for our newly launched Safe Options Support (SOS) team. This is an exciting opportunity for a Care Manager who is looking to transform community healthcare in Tioga and Tompkins counties and make long lasting, positive changes in the lives of homeless New Yorkers. As a Care Manager you’ll have the opportunity to:
- Enhance daily living skills
- Provide accompaniment to appointments
- Advocate for participants to ensure appropriate care
Requirements:
- Bachelor’s degree or higher, preferable in psychology, social work, sociology, or related field or be a New York State Licensed Practical Nurse (LPN).
- Previous case management experience in a social service agency, preferable serving a behavioral health population. Four years of past case management work experience may be considered in lieu of bachelor’s degree.
- Knowledge of homeless resources in Tioga and Tompkins counties a plus
- Proficiency in Health Information Technology and Microsoft applications
Responsibilities:
- Conducting outreach and engaging with homeless individuals
- Assessing health and social needs
- Assisting with hospital discharge planning
- Provide appointment navigation
- Monitoring progress and attending team meetings and supervisory sessions
- Perform other related duties as assigned

Work Location: In person throughout Tioga and Tompkins counties. This roll may require periodic on-call coverage and a willingness to work flexible hours
