Supervising Case Manager- $20.65-$30.97/hour
Pro Action’s Yates Office for Aging program is accepting applications for a Supervising Case Manager. The manager will supervise case management and housekeeping staff, handle contracts and referrals, assess eligibility, complete documentation, conduct home visits, assist with care planning and applications, and maintain accurate records in required databases. The manager will also oversee program and subcontractor budgets, process invoices, prepare contracts, and request budget changes as needed.
Bachelor’s degree in human services, social work, psychology or equivalent with experience in a related field of work with 3-5 years’ experience in related field of work. The candidate should have experience working with database and tracking systems as well as a variety of virtual platforms. Strong organization and computer skills and competent math skills are encouraged.
The ideal candidate must have a valid driver’s license, meeting agency standards per position and consistent, reliable transportation that will allow regular travel in and around the service area.
