Housing Program Case Manager

Qualifications
- Bachelor’s degree in Social Work, Human Services, or a related field, or a combination of equivalent education and experience.
- Prior experience in case management or providing direct services to individuals experiencing homelessness preferred.
- Experience working in mental health or substance use fields is a plus.
- Familiarity with data systems such as ServicePoint or similar case management platforms preferred.
- Must possess a valid Illinois Driver’s License and reliable transportation.
Primary Responsibilities
- Client-Centered Case Management
- Provide personalized case management to clients participating in Assigned Housing Program (Rapid Rehousing, Permanent Supportive Housing, Transitional Housing) focusing on securing stable housing, achieving income stability, and addressing basic needs.
- Collaborate with clients to develop and implement individualized service plans, conducting regular assessments to monitor and adjust goals as needed.
- Conduct weekly check-ins with clients to provide guidance, assess progress, and maintain engagement.
- Collaboration and Resource Building
- Establish and nurture partnerships with community organizations, government agencies, healthcare providers, and social service networks to enhance client outcomes.
- Advocate for clients by making referrals to appropriate services and resources, including employment programs, counseling, and healthcare providers.
- Conflict Resolution and Crisis Intervention
- Use effective conflict resolution strategies to manage challenges among program participants and staff.
- Respond promptly and effectively to emergencies involving clients or staff to ensure safety and resolution.
- Accurate Record-Keeping and Reporting
- Maintain thorough and timely documentation in client records using the ServicePoint system or other tools.
- Complete case notes, progress reports, and other required documentation in compliance with organizational standards and funder requirements.

- Perform other duties as assigned to support the success of the Assigned Housing program and the overall mission of Fifth Street Renaissance.
