As a Certified Community Behavioral Health Clinic, CNS Healthcare's mission is to serve the people of our communities.

The Intellectual and Developmental Disability (IDD) Supports Coordinator facilitates, coordinates, and monitors services for children, adolescents, and adults with intellectual and/or developmental disabilities and their support systems to ensure that the needs and wishes of the individuals are addressed and life opportunities are expanded. The core element of this position includes (1) assessments; (2) individual and family person–centered service plan development; (3) linking and coordinating services: (4) re-assessment and follow-up; and (5) ongoing monitoring services.

Compensation Range:

The starting pay for this position is $50,830 - $70,542 for limited-licensed candidates and $52,593- $75,315 for fully licensed candidates, based on non-discriminatory factors such as skills and experience.

  • Complete assessments to identify each person’s needs and desires which include the use of formal and informal tools to gather information needed to the person to identify their needs and wishes.
  • Perform comprehensive assessments, diagnosis, and evaluations.
  • Serve as an advocate to and provide supports coordination to children, adolescents, and adults with intellectual and/or developmental disabilities to facilitate, coordinate, and monitor supports and services using person-centered planning principles.
  • Enhance continuity of services with multiple providers and external partners, including, but not limited to the Department of Health and Human Services, schools, and vocational services.
  • Participate in developing person-centered planning meetings and IPOS treatment plans.
  • Support the development of coping skills.
  • Support the enrollment and obtainment of eligible benefit programs including Social Security Insurance and Medicaid benefits.
  • Train the individual and their support system on their rights and due process.
  • Consult with or provide consultation to other doctors, therapists, or clinicians regarding the individual’s care.
  • Promote coordination of care by keeping the treatment team informed of individual’s progress and current level of functioning.
  • Provide and participate in team consultation as assigned.
  • Participate in agency focus groups, work groups, and committees as assigned.
  • Support the development of agency staff and community partners by providing training as assigned.
  • Utilize motivational interviewing strategies and a trauma-informed approach to engage with and provide service to individuals.
  • Complete all required documentation completely, accurately, and in accordance with CNS
  • Meet direct service productivity and documentation standards per agency and program expectations.
  • Maintain compliance with quality care initiatives including Meaningful Use (MU), Medicare Merit Based Incentive Payment System (MIPS), Commission on Accreditation of Rehabilitation Facilities (CARF), and Certified Community Behavioral Health Clinic (CCBHC) requirements.
  • Ensure that the CNS Healthcare standards of service are applied to interactions with individuals served, guests, and staff.
  • Assume responsibility for professional growth by current reading and attendance at educational programs and conferences.
  • Maintain knowledge and compliance with established policies and procedures, corporate compliance program, code of ethics, applicable federal, state, and local laws and regulations, HIPAA standards, and other regulatory programs.
  • Perform other related duties as assigned.

Qualifications:

Required

  • Qualified Intellectual Disability Professional (QIDP)
      • Bachelor’s degree in Social Work, OR a Master’s degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy, OR
      • State licensure: Limited-Licensed Bachelor’s Social Worker (LLBSW), Licensed Bachelor’s Social Worker (LBSW), Limited-Licensed Master’s Social Worker (LLMSW), Licensed Master’s Social Worker (LMSW), Limited-Licensed Professional Counselor (LLPC), Licensed Professional Counselor (LPC), Limited-Licensed Marriage and Family Therapist (LLMFT), Licensed Marriage and Family Therapist (LMFT), Temporary Limited-License Psychologist (TLLP), Limited-Licensed Psychologist (LLP), OR Licensed Psychologist (LP)

      AND

      • Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons with intellectual or development disabilities as part of that experience), OR
      • At least 1 year of experience in training or working with a person who has an intellectual disability
  • Either currently possess or obtain the following professional designations within 1 year of employment: Child Mental Health Professional (CMHP) and Qualified Mental Health Professional (QMHP)
  • Valid and unrestricted Driver’s License
  • Transportation or Automobile accessibility with current automobile/vehicle insurance meeting agency policy requirements
  • Proficient with computers, Electronic Health Record (EHR) systems, and Microsoft office products including Word, Outlook, PowerPoint, Excel, SharePoint, and Teams.
  • Experience with the application of the Health Insurance Portability and Accountability Act (HIPAA) in health care organizations
  • Ability to work closely with persons with mental or physical limitations
  • Ability to format documents and use proper grammar, punctuation, and spell check.
  • Ability to work well as part of a multi-disciplinary team, communicate effectively, work collaboratively, and build positive working relationships
  • Ability to use discretion and judgmental when handling matters of a sensitive or confidential nature
  • Advanced time management, problem solving, customer service, interpersonal, and conflict resolution skills
  • Ability to be forward thinking and take initiative to accomplish goals and objectives of the department
  • Ability to contribute to an inclusive environment that recognizes the value and contributions of all persons regardless of race, ethnicity, national origin, gender, religion, age, marital status, sexual orientation, gender identity, or disability

Preferred

  • Prior experience working with all age groups including children; solution focused brief therapy; substance use and co-occurring disorders treatment; stages of change and/or motivational interviewing.
  • Prior experience in a community mental health setting.
  • Working knowledge and understanding of intellectual and developmental disabilities, co-occurring mental illness or other behavioral health disorders, medications used in treating behavioral health conditions, and experience with other interventions used to treat behavioral health conditions.
  • Ability to share and teach about the symptoms of substance use disorders that are similar to those of other medical and/ or psychological disorders and how these disorders interact
