Responsibilities:
As an Elder Care Team Coordinator, you will play a vital role in supporting older adults and their families by coordinating care and facilitating smooth team communication. Your responsibilities include:
- Promoting effective collaboration and communication within the elder care team and across the organization
- Supporting quality care delivery and helping improve the lives of seniors and their families
- Assisting the team with daily task management, visit coordination, and addressing patient needs
- Providing direct administrative support related to clinical operations for elder care services
- Offering compassionate support to elders and families during challenging times
- Preparing and coordinating clinical work schedules and calendars for the care team
- Managing incoming patient-related phone calls and coordinating with physicians, families, and staff
- Tracking team information including hours worked and time-off requests
- Collecting clinical data, assisting with reporting, and supporting quality metrics
- Organizing meetings, orientations, training, and safety updates, often collaborating with IT and other support staff
- Maintaining accurate clinical notes, health records, and data management systems
- Serving as a calming and empathetic presence for elders and their loved ones
- Encouraging and motivating the team to provide compassionate, person-centered care
- Assisting with other administrative tasks as needed to support the elder care team
- Associate or bachelor’s degree preferred, or equivalent professional experience combined with coursework
- Minimum four years of general office/administrative experience OR two years plus an associate degree in business or related field
- At least one year of experience in a healthcare or elder care setting, preferably with scheduling experience
- Proven ability to work independently, manage multiple projects, and maintain confidentiality
- Strong critical thinking, problem-solving, and sound judgment skills
- Proficiency in Microsoft Word and Excel; experience with EHR (Electronic Health Records) systems preferred
- Excellent verbal and written communication skills with the ability to work cooperatively with diverse individuals
- Respectful of different beliefs, choices, and lifestyles
- Positive attitude, initiative, adaptability, fairness, dependability, and teamwork orientation
- Ability to handle sensitive information discreetly
- Valid California driver’s license and reliable, insured vehicle
