Position Summary:
We are seeking a compassionate and experienced Weekend Hospice Admissions Nurse on Friday-Sunday, to guide patients and families through the admissions process with sensitivity and clinical expertise. This role is critical in evaluating prospective patients for hospice eligibility, providing education on hospice services, and ensuring a smooth and supportive transition to care.
Key Responsibilities:
- Perform clinical assessments to determine hospice eligibility based on established guidelines.
- Explain hospice philosophy, services, and care plans to patients, families, and caregivers with empathy and clarity.
- Complete all necessary admission paperwork and documentation accurately and in a timely manner.
- Collaborate with physicians, referral sources, and interdisciplinary team members to facilitate seamless admissions.
- Address patients’ and families’ questions and concerns, providing education and emotional support during the decision-making process.
- Coordinate and schedule the start of care with the hospice team, ensuring all necessary services are in place.
- Maintain up-to-date knowledge of hospice regulations, policies, and procedures.
- Participate in quality improvement initiatives and continuing education programs as required.
Qualifications:
- Current Registered Nurse (RN) license in Colorado.
- Minimum of two (2) years of clinical nursing experience; hospice, palliative care, or home health experience strongly preferred.
- Knowledge of end-of-life care practices, Medicare/Medicaid guidelines, and hospice admission criteria.
- Excellent communication, interpersonal, and customer service skills.
- Ability to work independently while being a strong collaborator within a multidisciplinary team.
