Job description

Community Care is hiring a Case Manager/Care Manager to join our PACE Program team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Position based in office at the Racine site, 5200 Washington Ave., Racine, WI 53406.

Care Manager/Case Manager

Responsibilities

Manages an assigned caseload of program members

Completes comprehensive member assessments, member centered care plans, & functional screens

Provides ongoing care coordination and quality assurance monitoring of services

Maintains required documentation in adherence to established standards.

Care Manager/Case Manager

Requirements

·Education: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree. Bachelor's Degree in Social Work preferred

·Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.

·Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Care Manager Job Schedule:

Monday to Friday, 8:00 AM-4:30 PM
