❤️ About the Role

As a Case Manager, you will play an important role in helping adults with developmental disabilities access the services, resources, and support they need to achieve their personal goals.

You will work directly with individuals, families, service providers, and professionals to develop and coordinate individualized plans that promote independence, wellness, autonomy, and community participation.

This is a mobile, community-based position. You will conduct face-to-face visits and may travel to client locations throughout the region.

What You'll Do

  • Coordinate individualized services and resources based on each client's needs and goals
  • Conduct face-to-face home and community visits
  • Complete and update client assessments and service plans within established timelines
  • Monitor client progress and review plans, reports, and supporting documentation
  • Work collaboratively with clients, families, healthcare professionals, social workers, and service providers
  • Identify barriers and develop practical solutions to support positive outcomes
  • Adjust service plans when circumstances or client needs change
  • Maintain accurate, timely, and confidential case records
  • Help clients access appropriate community resources and services
  • Promote client choice, independence, dignity, and self-sufficiency
  • Ensure services and documentation remain consistent with organizational standards and applicable requirements
  • Identify opportunities for improvement and contribute to quality outcomes
  • Manage multiple cases while prioritizing deadlines and client needs

📍 Travel & Work Environment

This is a full-time, community-based position. Regular travel to client locations is required.

Depending on business and client needs, assignments may include:

Candidates must be comfortable traveling throughout the assigned service area.

A company vehicle may be provided for qualifying long-distance assignments.
