Case Manager/Behavioral Health Specialist, Children's Clinic

Valley Health Partners, Allentown, PA

Summary

Completes comprehensive assessments and works collaboratively with clients to create care plans based on identified needs. Assists patients and immediate family members in accessing and navigating benefits. Coordinates care and places referrals to clinical providers and appropriate community resources.

Provides a full range of mental health outpatient services including initial assessment, treatment planning, individual and group psychotherapy, suicide, or other risk assessment, as well as crisis intervention, case management, and discharge planning.

Qualifications

Experience: Two years of experience in clinical/ambulatory care setting required

Education: Master's degree in social work, Psychology or other related field required

Licenses and Certifications: LPC or LCSW required

Skills needed :

  • Ability to work efficiently with people from a broad range of backgrounds.
  • Proficient computer skills and ability to use the Electronic Health Record to maintain timely, accurate documentation and effective communication with clients, team members, and external partners.
  • Ability to adhere to the National Association of Social Workers, or American Counseling Association or Code of Ethics. (Based on Licensure)
  • Knowledge of patient rights and laws relative to those rights, such as HIPAA.
  • Knowledge of psychological principles and theories.
  • Must possess a high degree of emotional intelligence, and empathy towards others.
  • Skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
  • Excellent verbal and written communication skills.

Essential Functions:

Care Management responsibilies:

  • Completes same-day & scheduled, intakes and assesses the psychosocial and socioeconomic needs and barriers of care for patients/clients/families.
  • Participates as an integrated member of the client’s care team.
  • Develops individualized care plans based on assessed needs.
  • Ensures accurate and timely documentation of all client interactions in the Electronic Health Record for effective communication with clients, team members, and providers.
  • Efficiently manages referrals from internal and external sources, ensuring timely coordination and continuity of care.
  • Coordinates with community partners to ensure access to care and services.
  • Provides education on available resources and empowers patients/clients/families to achieve optimal levels of wellness and independence.
  • Participates in community outreach and engagement events.
  • Assists leadership with process improvement and program evaluation.
  • Prepares and delivers outreach presentations, coordinates events, and promotes awareness of VHP programs/services.

Behavioral Health Responsibilities

  • Provides culturally sensitive individual, family, and group psychotherapy in support of the patient's treatment plan monitoring progress toward goals and adjusting treatment approaches, as indicated.
  • Provides psychosocial assessments including clinical impressions and treatment plan, in collaboration with the patient.
  • Complete billable behavioral health consultations, and intakes and follow up visits, as warranted based on clinical need
  • Monitors for and addresses suicide/high risk as indicated by professional and departmental standards
  • Provides timely discharge planning with a grasp of community supports; educates patient and facilitates referrals based on treatment needs.
  • Will meet departmental productivity standards based on cFTE (clinical FTE)
  • Performs other duties as assigned.
