Case Manager II - Benefits & Eligibility

  • Reports To: Program Manager - Housing Stability, North or South
  • Work Schedule & Location: This is an in-office position, with flexibility for occasional remote work or adjustments to daily schedule as operational needs allow. Core working hours fall between 8:00 a.m. and 6:00 p.m., with flexibility in start and end times.

Position

Overview

The primary objective of the Case Manager II - Benefits & Eligibility is to help Minneapolis Public Housing Authority (MPHA) residents access benefits, complete required MPHA recertification processes and income change updates, connect to income supports and community resources, and receive individualized services that strengthen housing stability and overall well-being.

This position provides resident-centered case management, benefits navigation, and recertification support across assigned MPHA properties. The Case Manager works directly with residents to identify needs and goals, understand eligibility and recertification requirements, complete benefit applications and renewals, prepare for annual and interim MPHA recertifications, support required income change updates, gather verification documentation, resolve barriers, and connect to longer-term community supports. The role also works collaboratively with MPHA staff, Agate team members, resident leaders, and community partners while maintaining appropriate confidentiality and resident choice.

Position

Responsibilities

  • Provide resident-centered case management to MPHA residents, with an emphasis on benefits access, income stability, and housing stability.
  • Meet with residents to identify needs, strengths, goals, and potential eligibility for public benefits, income supports, and community assistance programs.
  • Assist residents with benefit applications, renewals, MPHA annual and interim recertification requirements, and required income change updates, including gathering verification documents, completing resident portions of required forms, tracking deadlines with follow-up and problem solving when barriers or delays occur.
  • Help residents understand benefit and MPHA recertification notices, income change reporting requirements, requests for verification, and requested next steps, and connect them with the appropriate MPHA staff, public agency, community provider, legal resource, or advocacy support when additional assistance is needed.

  • Develop individualized service goals and follow-up plans with residents, using warm referrals and coordinated handoffs whenever possible.
  • Provide regular on-site office hours, outreach, and individual meetings at assigned MPHA properties, including in-home visits when appropriate and requested.
  • Support residents in addressing barriers that may affect housing stability, including income, food access, transportation, utilities, health coverage, and other basic needs.
  • Provide education and resources related to tenant responsibilities and rights, financial stability, and available community services.
  • Build and maintain working relationships with benefit agencies, community organizations, and service providers to improve resident access to resources.
  • Work cooperatively and collaboratively with MPHA staff, Agate staff, resident leaders, and community partners to coordinate services and respond to resident needs.
  • Complete accurate case notes, data collection, referrals, service tracking, and other required documentation in a timely manner.
  • Maintain resident confidentiality and follow Agate, MPHA, funder, and program requirements related to documentation, information sharing, and service delivery.
  • Travel between assigned MPHA properties and perform all other duties as assigned.

Required

Qualifications

  • Bachelor's degree in social work, human services or related field, or minimum 2 years of experience in similar role.
  • Experience providing person-centered services and supporting individuals in navigating complex systems and community resources.
  • Ability to manage multiple priorities, work independently across community-based sites, and maintain accurate and timely documentation.
  • Proficiency with Microsoft Office Products.
  • Excellent problem solving and conflict resolution skills.
  • Proficiency in multi-tasking while maintaining accuracy.
  • Must have valid MN Driver's License and pass background check.
  • Demonstrated ability to network via email, telephone, and in person to advocate for participants
  • This position requires use of your own personal vehicle and will require insurance liability coverage of at least one hundred thousand dollars per person and three hundred thousand dollars per occurrence and must have a valid Driver's License and pass a driving record background check.

Qualifications

  • Experience working in public housing, affordable housing, resident services, or housing stability programs.
  • Experience helping individuals navigate public benefits and income-support programs such as food, health coverage, cash, disability, energy, or other assistance programs.
  • Experience working with diverse resident populations and providing services that are culturally responsive, accessible, and grounded in resident choice.
  • Experience networking and building collaborative relationships with community organizations and public systems.
  • Bilingual or multilingual skills preferred but not required.
  • Knowledge of Housing First, Harm Reduction, trauma-informed, and person-centered service principles.
