  • Reports To: Program Manager - Housing Stability, North or South
  • Work Schedule & Location: This is an in-office position, with flexibility for occasional remote work or adjustments to daily schedule as operational needs allow. Core working hours fall between 8:00 a.m. and 6:00 p.m., with flexibility in start and end times.

Position

Overview

The Case Manager II - Housing Stability provides housing-focused, resident-centered case management to individuals and families living in Minneapolis Public Housing Authority (MPHA) communities. The position builds relationships with residents, identifies barriers to housing stability, coordinates resources, and provides timely support to help residents maintain their housing and achieve self-directed goals.

The Case Manager II - Housing Stability will be dedicated to an assigned MPHA Asset Management Project (AMP) and its associated properties, serving residents in senior and disability housing, family housing, mixed-population communities, or scattered-site properties.

While core responsibilities remain consistent across assignments, day-to-day service priorities and resident engagement approaches will be tailored to the unique needs of each community. Assignments will be determined based on resident needs, relevant experience, candidate interests, and organizational staffing priorities.

Position

Responsibilities

  • Build trusting, professional relationships with residents through proactive outreach, regular check-ins, and accessible, voluntary service engagement.
  • Assess resident-identified needs and strengths; develop, implement, and update individualized housing stability and service plans with residents.
  • Provide eviction prevention and housing stabilization support, including early intervention around lease compliance concerns, rent or income changes, and connections to available resources.
  • Assist residents with benefits and resource navigation, including referrals for income supports, employment, healthcare, food, transportation, and other community services.
  • Coordinate with MPHA property management to address resident concerns and prevent housing loss while maintaining appropriate professional boundaries and confidentiality.
  • Provide crisis response, de-escalation, problem-solving, and referrals to appropriate internal or community-based services as needed.
  • Educate residents about lease obligations, tenant rights and responsibilities, and ways to communicate concerns with property management.
  • Collaborate with Agate colleagues, specialized case management positions, and community providers to connect residents to appropriate services without duplicating specialized roles.
  • Provide services at assigned properties and, as appropriate to assignment, through scheduled home visits and community-based outreach.
  • Complete accurate case notes, service data, follow-up, and other required documentation in a timely manner; meet program and funder reporting expectations.
  • Participate in team meetings, professional development, and coordinated cross-coverage across MPHA properties when needed.
  • Perform other related duties as assigned.

Assignment Opportunities and Service Emphases:

A Case Manager may serve one or more properties or AMPs. The descriptions below illustrate potential areas of emphasis; they do not create separate job classifications or require every Case Manager to possess every specialty.

  • Senior and disability housing: Support aging in place, independence, health and community-service connections, and coordination with specialized benefits or waiver staff.
  • Family & Scattered-site Housing: Support households with dependent children through family stabilization, school and childcare connections, income supports, and family-oriented community resources. Provide site based and scattered-site home-based outreach across dispersed properties;
  • Mixed-population and high-rise communities: Emphasize proactive engagement, housing stabilization, crisis coordination, and accessible services for residents with varied needs.

Required

Qualifications

  • Bachelor's degree or minimum 2 years of experience in a similar case management, housing, or human services role.
  • Ability to build effective relationships with residents of diverse ages, backgrounds, household compositions, and service needs.
  • Strong problem-solving, communication, advocacy, and conflict-resolution skills.
  • Ability to manage multiple priorities, work independently, collaborate with a team, and maintain accurate documentation.
  • Proficiency with Microsoft Office products and ability to learn required data and case management systems.
  • Ability to travel to assigned MPHA properties and community locations as needed; must meet applicable driving and background screening requirements for the assignment.
  • Must have valid MN Driver's License and pass background check.
  • Demonstrated ability to network via email, telephone, and in person to advocate for participants.
  • This position requires use of your own personal vehicle and will require insurance liability coverage of at least one hundred thousand dollars per person and three hundred thousand dollars per occurrence and must have a valid Driver's License and pass a driving record background check.

Preferred

Qualifications

  • Experience in public housing, resident services, eviction prevention, housing stabilization, or related community-based services.
  • Experience with one or more populations served through MPHA assignments, including older adults, people with disabilities, families with dependent children, or residents with complex service needs.
  • Experience coordinating with property managers, community providers, and interdisciplinary teams.
  • Bilingual skills preferred but not required.
  • Knowledge of Housing First and Harm Reduction principles.
